Medication Management in Primary Care
Saint Luke’s medication management pharmacists work collaboratively with the primary care team to optimize patients’ medications and fine-tune them over time. Our team works with patients to implement lifestyle changes such as exercise and nutrition that may reduce the number of medications required.
Our team’s goal is to ensure every medication is working to improve a patient’s overall health, is safe, and affordable.
Diabetes program
Our pharmacists provide patients with diabetes with education, coaching, and medication management to meet their health goals and promote disease reversal. We work with patients over time to adjust medications, providing follow-up and coordination with the care
team. We help patients achieve their A1C goal, lose weight, feel better, and afford their medications.
What to expect
Visits with the medication management team can be done in person, by video, or by phone.
- Bring a list of medications for review with the pharmacist
- Be prepared to discuss health goals and any barriers
- We will develop a plan with specific actions needed to achieve goals
- We will update the patient’s care team members after each visit
