Health Information Exchanges (HIE)
Saint Luke’s Health System participates in various electronic health information exchanges designed to ensure that patient health information is readily available to all persons, entities, payors, and others permitted by law, who are involved in providing care. Our participation is described in Saint Luke’s Health System Consent and Agreement of Health Care Services (Spanish version). These exchanges, and the health care organizations each includes, are listed below:
The intent of these exchanges is to provide caregivers with secure access to patient health information so they can render the most effective care regardless of where the patient seeks that care. This access is automatic per regulatory guidelines. However, patients may choose to opt-out of providing this access by sending a request to firstname.lastname@example.org using the subject line “HIE Opt-Out.” Please include Patient Full Name, Date of Birth, and Phone Number so we can verify the request. To revoke this opt-out request, send a request to email@example.com using the subject line “Revoke HIE Opt-Out”. Please include Patient Full Name, Date of Birth, and Phone Number so we can verify the request.
NOTICE: If you send health information to Saint Luke’s Health System by email, please know that your message may be sent in an unencrypted email. An unencrypted email means there is a risk that the information in the email and any attachments could potentially be read by a third party when it is sent through the internet. By sending your request by email, you are agreeing to accept these risks.
