Skilled Nursing Facilities
A skilled-nursing facility provides post-hospital, transitional care. Typically, a skilled-nursing facility is a temporary residence for patients undergoing medically necessary rehabilitation. The goal for patients in skilled nursing is to get well enough to go home. Skilled nursing care includes services such as administration of medications, tube feedings, wound care, and additional support as the patient recovers from illness, injury, or surgery.
Medicare Part A may cover skilled-nursing care if:
- Patient was admitted as an inpatient at a hospital for at least three consecutive days (three midnights)
- Patient enters a Medicare-certified skilled-nursing facility within 30 days of leaving the hospital and receives care for the same condition the patient was treated for during the hospital stay
- Patient requires skilled nursing care seven days per week or skilled therapy services at least five days per week

Home health care is a wide range of health care services that can be given in your home for an illness or injury. Home health care is usually less expensive, more convenient, and just as effective as care you get in a hospital or skilled nursing facility (SNF). In general, the goal of home health care is to treat an illness or injury.

- Wound care for pressure sores or a surgical wound

- Intravenous or nutrition therapy
- Injections
- Monitoring serious illness and unstable health status

Long-term Acute Care Hospitals
Long-term acute care hospitals are facilities specializing in the treatment of patients with serious medical conditions requiring ongoing care but no longer require intensive care or extensive diagnostic procedures. These patients are typically discharged from an intensive care unit and require more care than they can receive in a rehabilitation center, skilled-nursing facility, or at home.
What types of patients are seen in a long-term acute care hospital?
According to Medicare guidelines, the patient must need more than 25 days of hospitalization. The average length of stay of a person in a long-term acute care hospital is approximately 30 days.
The types of patients typically seen in a long-term setting include those requiring:
- Prolonged ventilator use or weaning
- Ongoing dialysis for chronic renal failure
- Intensive respiratory care
- Multiple IV medications or transfusions
- Complex wound care or care for burns
