Health News You Can Use: Social Drivers of Health and Community Health Initiatives
Research shows about 80% of a person’s health is related to the conditions where they are born, live, learn, work, and play—also known as social drivers of health. Because of this, Saint Luke’s has a special team dedicated to connecting patients with resources to meet their needs beyond the walls of our hospitals and clinics.
This week we are talking to Meredith Cantrell, program manager of Community Health Initiatives at Saint Luke’s, about how Saint Luke's uses social drivers of health to help patients in our community.
