Student Placement at Saint Luke’s
Saint Luke’s values the partnership between education and health care, and both fields can help the other excel. Our student placement program offers students the opportunity to gain practical experience and develop the necessary skills for a successful career in the health care industry.
Students interested in the placement program must fill out the following forms:
- Pre-submission Checklist: Used by the school and student to ensure all documents are complete before submission. Also includes important information and resources to support your clinical experience.
- Computer Access Request Spreadsheet: Used to request IT access within our Electronic medical Record (EMR). Please download, complete, and return this spreadsheet via email to [email protected]. The form will not be submitted automatically. Note that IT access may take up to four weeks after form submission.
- Clinical Onboarding Requirements: Use this checklist to ensure you have requirements and paperwork completed.
- School Attestation Form: Signed by the school to confirm student has met all listed requirements. Cohorts may list all students on this form with one signature.
- Student Participation Authorization: This should be signed by the student.
- Tuberculosis (TB) Risk Assessment Form: Students attending clinicals must complete page 1 page the Missouri Department of Health and Senior Services TB Risk Assessment Form annually. If a student answers “YES” or leaves any field blank (“NO RESPONSE”) in Sections A or B, they are also required to complete Section C and undergo a Medical Evaluation.
- The Saint Luke’s Privacy and Security training and quizzes are not required if you are part of MOKAN or an employee. Others are required to complete the training and quizzes.
