This form will be used to create your Find a Provider profile on the Saint Luke’s website. Find a Provider is an important directory used by patients to learn more about you.

Once you have filled out this form, the information will be verified and posted to your profile. The information you provide here will also be used to develop marketing materials to promote you internally and to patients.

List academic titles from Saint Luke’s Health System partner teaching institutions, including University of Missouri-Kansas City.
Please list, in priority order, your areas of clinical interest. Some of these terms may be used in your bio and printed marketing materials, but many will not be visible on the website. They will be used to build keywords for website search purposes.
