Saint Luke’s Care Choices is a Centers for Medicare and Medicaid Services (CMS) program that encompasses all aspects of care—palliative care, end-of-life care, and 24/7 on-call support services—for patients who are pursuing curative treatments. Like palliative and hospice care, Care Choices offers care coordination, symptom and pain management, and promotes patient and family preferences and shared decision-making. Like hospice, Care Choices focuses on quality of life at the end of life; but unlike hospice, it allows continued Medicare payment for continued aggressive treatment.

Saint Luke’s is proud to be the only hospice service provider in the Kansas City metro area selected by CMS to take part in this innovative pilot program. Care Choices is designed for patients in Missouri who have a diagnosis of advanced cancer, end-stage COPD, congestive heart failure, or HIV/AIDS.

Fewer than half of eligible Medicare beneficiaries use hospice care and most only for a short period of time. The Saint Luke’s Care Choices program is designed to:

  • Increase access to supportive care services
  • Improve quality of life and patient and family satisfaction
  • Allow patients to continue pursuing an aggressive plan of treatment
